- You forgot to put Country into Application Form
- State is mandatory and I think it shouldn't. It refers only to US.

- I think MSN/ICQ should not be mandatory.
For example I use only skype and I put none in application form for MSN/ICQ
- Country should be as a drop down list not as a free text value
- I would use name instead of lastname

Changes:
1. There is a dropdown menu for countries now.
2. ICQ/MSN isn't required anymore.
3. The field "Lastname" is called "Name" now!

I don't know if only I have this problem, but I haven't received confirmation e-mail after registering using Application form. I filled in form yesterday's evening.
